Health Information Management

The Health Information Management (HIM) Department of the Sac and Fox Nation (SFN) Black Hawk Health Center (BHHC) will maintain the confidentiality, security, legibility, and accessibility of patient health records.

To request personal medical health records from BHHC, the patient must complete a Medical Release of Information Form. For questions regarding Release of Information, please call the HIM Department at 918-968-9531 x3028 or x3027

Medical

It is the goal of the Medical department to provide the best possible health care to our patients.

This department manager is an M.D.

The medical department normally consists of three full time medical providers. These providers all have professional Degrees and postgraduate training, followed by years of experience practicing medicine. Most have a lot of experience in Native American health issues.

Lab

It is the goal of the laboratory department to provide quality care, accurate test results, and definitive diagnostic information for our patient population and health care providers.

The department manager is a Registered MLT.
The Lab department performs in-house and send-out labs testing.

The In-House lab is resulted within 1 hour for the convenience of the physicians and patients.

Specialty labs are sent to a reference laboratory and the results are sent back to Black Hawk Health Center via fax when completed. The results are then put into the Electronic Health Record for the physicians.

Behavioral Health

It is the goal of the Behavioral Health Services department to promote healthy lifestyles and provide a quality of care that enhances the lives of the Native population and families living in our communities.
We embrace a holistic approach of body, mind and spirit that facilitates self-empowerment, prevention, education, and intervention. We offer assistance to clients in learning to cope with chemical dependency, mental health and/or co-occurring disorders, or other behavioral health disorders, including trauma, difficult life transitions, grief or other interpersonal difficulties for wellness and recovery.
The staff are licensed and credentialed. All services are provided on an outpatient basis. All information is kept strictly confidential.

Business Office

It is the goal of the Business Office department to perform quality timely filing of insurance with accountability and accuracy.
The Business Office is responsible for billing your insurance. Please keep us informed with your insurance changes. When we receive payment, that money is put back into the clinic to better serve you and your needs.
The deductible and co-pays are written off. We would never bill you for the balance due.
If you need to be sent to a specialist through Purchased/Referred Care, using your insurance at the clinic helps with the balance due that Purchased/Referred Care would have to pay.

Nursing

It is the goal of the Nursing department to provide quality nursing care to the patients.

The department manager is a Registered Nurse.

There are four Licensed Practical Nurses that are responsible for screening the patient, doing the vital signs, checking for allergies, checking immunization status, and medication lists prior to the patient seeing the physician. Also give injections as ordered by the physician. They assist the physicians with procedures and treatments. Assist the patients with any questions they may have about their visit.

The nursing staff receive many phone calls. When the staff is in exam rooms taking care of a patient, you may have to leave a message for them to call you back.

Nursing staff check the lab orders prior to the appointment to make sure the tests ordered are what the physician still wants for the coming visit.

They ensure that lab results and x-ray reports ordered by outside providers are faxed to them upon completion of the tests.

Radiology

It is the goal of the radiology department to perform services with optimum accuracy, precision, efficiency and safety.
The department manager is an AART Registered Radiographer.
The Radiology department provides Digital Imaging Service, the most advanced technology available.
The department uses teleradiology with an off-site radiology group. Utilizing this technology, the radiologist report is commonly available in the Electronic Health Record within 24 business hours.

The physicians are able to view our patients x-rays from computer workstations located throughout the clinic and Merle Boyd Center. Although the radiology department is now “filmless” and “paperless”, the patients can be provided with a CD of their x-ray images for referral purposes.

Within the Black Hawk Health Center we also have other services:

Purchased/Referred Care (PRC) 

The Contract Health Services department has now been renamed to Purchased/Referred Care (PRC) department. It is the goal of the Purchased/Referred Care department to find qualified health care professionals to continue the care of our patients, and to process claims accurately and efficiently.

This program is funded under the Indian Health Services; Department of Health and Human Services, and covers contracted quality health/medical care for qualifying individuals.

Some services are not covered though Purchased/Referred Care. If you have any questions about what is covered or not covered please call the PRC department.

Eligibility

Home address and tribe are used to help determine if you are eligible for Purchased/Referred Care services.

Referrals

A patient is required to see a Black Hawk Health Center provider to request a referral for a specialist outside the clinic.  If the referral is approved for services the PRC staff will schedule the patient’s appointment and they will fax all the supporting information to the provider. PRC services will contact the patient by phone or mail about the appointment date and time.

ER and/or Admits

If you go to the Emergency room contact Contract Health within 72 hours. If you are admitted to the Hospital contact Contract Health within 72 hours. Make sure you sign a Release of Information at the Hospital so Contract Health can help get the records.

Physical Therapy

The Physical Therapy Department is staffed with two full-time physical therapists specializing in all musculoskeletal and orthopedic conditions from chronic joint or muscle pain to post-surgical care. Both therapists have Doctor of Physical Therapy degrees from accredited universities. The goal of the department is to return patients to their normal pain-free and active lifestyles.

Our physical therapists are movement specialists with 7+ years of university training in understanding the anatomy and kinesiology of human movement and performance.  Your therapist will find the area of dysfunction and may use gentle stretching, patient specific exercise, and manual “hands on” techniques to correct the movement disorder, reestablishing normal function of the body and reducing pain levels. Physical therapy can help to greatly reduce symptoms of back and neck pain, shoulder pain, knee pain, arthritis, muscle pain, ankle/foot pain, and a myriad of other aches and pains.

You must have a referral from your Black Hawk primary care provider to be eligible for services with the physical therapy department.  A typical visit consists of one-on-one personalized treatments lasting between 45-60 minutes.  Treatment frequency will vary per patient based on individual patient needs. Contact your Black Hawk care provider if you think you would benefit from physical therapy.

Exercise Consultation/Evaluation

A patient must have a referral from a provider at the Black Hawk Health Center before they are eligible to receive this service.

After the initial Consultation/Evaluation is completed the patient has the following options available.

  1. Have an individual work-out plan developed.
  2. Joining one or multiple exercise classes.
  3. Participating in an incentive based exercise activity.
  4. Combination of 1, 2, & 3.

A fully equipped exercise room is available Monday through Friday.

Dietitian Services

The nutrition department is staffed by a licensed Registered Dietitian who is certified by the Commission on Dietetic Registration. The dietitian holds a master’s degree in nutritional sciences and is also credentialed as a Diabetes Care and Education Specialist. You must have a nutrition referral from your Black Hawk primary care provider to be eligible for services.

It is the goal of the nutrition department to promote improved health and reduce the risk of developing chronic diseases by encouraging a healthy diet. The dietitian provides medical nutrition therapy focused on prevention and management of chronic diseases including heart disease, hypertension, diabetes, kidney disease, and obesity. The dietitian counsels patients on behavioral and lifestyle modifications required to improve health outcomes by providing individually-tailored nutrition recommendations.

Cooking demonstrations focusing on diabetes management, heart health, and weight loss are also provided monthly. Patients must sign-up with the dietitian to attend these classes – extension x3042.

Transportation

We have three transportation drivers.

If you are eligible for services at Black Hawk Health Center or Purchased/Referred Care, then you may request transportation assistance.

Your appointment must already be scheduled for Black Hawk Health Center, the Merle Boyd Center, Community Health department for IADL’s (Independent activities of daily living), or you have been referred through Purchased/Referred Care with an appointment for Medical, Dental, or Eye.

Transport must be scheduled a minimum of three (3) business days in advance.

Transportation is not guaranteed and scheduling is based on availability.

Community Health Workers (CHW)

It is the goal of the Community Health Worker (CHW) to provide quality outreach health care services, health promotion/disease prevention services, implement medical protocols to facilitate healing, identify and assist with unmet physical and psychosocial needs, and provide emotional support while acting as a patient advocate.

The department manager is a Registered Licensed Nurse—RN.

There are two Licensed Practical Nurses (LPN’s) and two Certified Nurse Assistants (CNA’s) that do home visits for patients that need a blood pressure check, medication box refill, dressing changes, and all-around advocate for the patient.

Patient Registration

Our Telephone Operator is responsible for assisting you when available to, or they will direct the call to the appropriate personnel or department who can assist you.

We are responsible for scheduling appointments for the following:  BHHC medical providers only, registration appointments to Establish Care or for referral purposes, nursing, lab/x-ray and for Same Day Access, formally known as walk-ins.

Registration is responsible for checking in patients for their appointment(s). We are also responsible for keeping your information current, which is very important in case we need to contact you about your appointment or lab/x-ray results, enter new insurance information, and to obtain signatures on annual basis paperwork.

We also have a Benefit’s Coordinator who will assist you in filing for alternate resources if you do not have any insurance coverage. The Benefit’s Coordinator is responsible for linking your patient portal after you’ve created your account.

Podiatry

Black Hawk Health Center offers a podiatry clinic each Wednesday. The podiatrist is certified by the American Board of Podiatric Medicine as a doctor of podiatric medicine (DPM). The podiatrist is a medical specialist who has been trained to assess, diagnose, and treat conditions related to the feet, ankle, lower limbs, and their associated structures.

Special Diabetes Program

In response to the diabetes epidemic among AI/AN people, Congress established the Special Diabetes Program for Indians (SDPI) in 1997. The SDPI provides funds for diabetes prevention and treatment services to IHS, Tribal, and Urban Indian communities across the United States.

SDPI funding supports many services at Black Hawk Health Center including the purchase of diabetic shoes, glasses, and testing supplies, the Diabetes Prevention Program, Joslin Vision Network, Eagle Adventure Program, cooking demonstrations, Sac and Fox health fair, Elder’s Olympics, summer reading programs, gardening efforts, Jim Thorpe Day, the Black Hawk Walk, and various community health efforts throughout the year.
